Greenwell PCN Abridged Access Plan

The seven practices in Greenwell PCN work closely together in order to achieve an equitable and high quality service to the 40,000 patients in the locality.

The PCN aims to ensure all registered patients can access appropriate Primary Care appointments with the correct clinician within reasonable time frames by reviewing appointment structure, clinicians and services available, working with health and social care providers supporting primary care within North West London.

Key areas

All patients will be triaged to ensure they receive an appointment with the correct member of the practice team. All practices endeavour to offer same day appointments where possible as detailed in the flow diagram.

All practices answer 90% of telephone calls within 10 minutes.

All practices respond to 90% of e-submissions by the end of the working day.

All practices are committed to increasing the number of patients using the NHS APP.

All practices are continuing to engage with patients via various forums

All practices have policies in place for our patients with Learning Disabilities, working hard to ensure their disability is not an obstacle to accessing health care.
