Learning Disabilities Access Policy
Patients with Learning Disabilities registered at any of the practices within Greenwell PCN can expect to be access health care with a focus on equity and respect.
Adjustments for these patients will be made to ensure the patient has an appointment with the most appropriate member of the practice team in a safe environment. The PCN works hard to ensure that Learning Disabilities are not an obstacle to accessing health care and will make reasonable adjustments wherever possible.
The patient can expect;
- A same day appointment (if this is not possible then next day).
- Not to be kept waiting – an alert on the patients notes ‘DO NOT KEEP WAITING’.
- For all health needs to be discussed and health checks made in the same appointment e.g. BP; weight check to avoid multiple appointments.
- Ensure that carer information is current and up to date.
- Where possible blood tests to be done outside of core appointment hours to provide a quiet and calm environment for the patient.
- Ensure that they have been understood and understand what has been discussed and what the treatment plan is and that their carer / companion are fully aware of what was discussed during the appointment.
- To be treated with respect.
The patients and their carers can contact the practice to discuss any concerns or issues to ensure that they can access primary care.
